Indications:
Baricitinib is a Janus kinase (JAK) inhibitor indicated for:
- The treatment of adult patients with moderate to severe active rheumatoid arthritis who have had an inadequate response to one or more TNF blockers.
Use Limitation: Not recommended for use in combination with other JAK inhibitors, biologic DMARDs, or potent immunosuppressants such as azathioprine and cyclosporine. - The treatment of hospitalized adult patients with COVID-19 who require supplemental oxygen, non-invasive or invasive mechanical ventilation, or extracorporeal membrane oxygenation (ECMO).
- The treatment of adult patients with severe alopecia areata.
Use Limitation: Not recommended for use in combination with other JAK inhibitors, biologic immunomodulators, cyclosporine, or other potent immunosuppressants.
Dosage and AdministrationAdministration Notes:
- Refer to the full prescribing information for recommended pre-treatment evaluations and immunizations.
- For rheumatoid arthritis and alopecia areata: Treatment should not be initiated or should be interrupted in patients with anemia (hemoglobin < 8 g/dL), lymphopenia (ALC < 500 cells/mm³), or neutropenia (ANC < 1,000 cells/mm³).
- For COVID-19: Treatment should not be initiated or should be interrupted in patients with lymphopenia (ALC < 200 cells/mm³) or neutropenia (ANC < 500 cells/mm³).
Recommended Dosage:
- Rheumatoid Arthritis:
2 mg orally once daily.
Baricitinib may be used as monotherapy or in combination with methotrexate or other non-biologic DMARDs. - COVID-19:
4 mg orally once daily for up to 14 days. - Alopecia Areata:
2 mg orally once daily. If response is inadequate, increase to 4 mg once daily.
For patients with near-complete or complete scalp hair loss, with or without significant eyelash or eyebrow hair loss, 4 mg once daily may be considered.
Once adequate response is achieved, reduce to 2 mg once daily.
